Book excerpt: Each year in the United States, more than 4,000 occupational fatalities and more than 3 million occupational injuries occur along with more than 160,000 cases of occupational illnesses. Incorporating patients' occupational information into electronic health records (EHRs) could lead to more informed clinical diagnosis and treatment plans as well as more effective policies, interventions, and prevention strategies to improve the overall health of the working population. At the request of the National Institute for Occupational Safety and Health, the IOM appointed a committee to examine the rationale and feasibility of incorporating occupational information in patients' EHRs. The IOM concluded that three data elements - occupation, industry, and work-relatedness - were ready for immediate focus, and made recommendations on moving forward efforts to incorporate these elements into EHRs.

The problem is an informational supply chain that is unnecessarily dependent on the minds of doctors for assembling patient data and medical knowledge in clinical decision making. That supply chain function is more than the human mind can deliver. Yet, dependence on the mind is built into the traditional role of doctors, who are educated and licensed to rely heavily on personal knowledge and judgment. The culture of medicine has long been in denial of this problem, even now that health information technology is increasingly used, and even as artificial intelligence (AI) tools are emerging. AI will play an important role, but it is not a solution. The solution instead begins with traditional software techniques designed to integrate novel functionality for clinical decision support and electronic health record (EHR) tools. That functionality implements high standards of care for managing health information. This book describes that functionality in some detail. This description is intended in part to be a starting point for developers in the open source software community, who have an opportunity to begin developing an integrated, cloud-based version of the tools described, working with interested clinicians, patients, and others. The tools grew out of work beginning more than six decades ago, when this book’s lead author (deceased) originated problem lists and structured notes in medical records. The electronic tools he later developed led him to reconceive education and licensure for doctors and other health professionals, which are also part of the solution this book describes.

Book excerpt: Changing healthcare regulations and rapid technological advancements have made the transition to electronic health records (EHRs) intensively challenging for healthcare institutions. As healthcare providers acclimate to the pressures of advanced EHRs, it is imperative to construct well-established documentation guidance and education that can assist health professionals in caring for their patients. Vast research indicates the problem list, a component of the EHR, is poorly maintained and lacks a standardized approach. The inconsistent use of the EHR problem list supports the need to characterize the existence of institutional EHR problem list guidance and education. These establishments can help supply a clear understanding of documentation requirements and the perception of its intended use. The purpose of this research study is to evaluate EHR problem list guidance, education, and general use across multiple institutions. This study uses a mixed-method approach of structured interviews and open-ended discussion with informatics leaders of healthcare institutions. Results reveal that institutions share a common approach to EHR problem list guidance and education; however, informatics leaders present some barriers which hinder their overall use in clinical practice, including complexities beyond their existence and enforcement.
